I would add to Mike's excellent list a few things from the medical
standpoint.
Get appropiate vacinnations: include Hepatitis A and B, Cholera, Small pox,
influenzia, Tetinus/diptheria, polio, Meningococcus, childhood diseases if
indicated.
Second, if planning a passage, I advise having a good comprehensive physical
exam.
This would include EKG, stress tests, blood tests and Xrays. Also a medical
statement for each member of the crew would be advisable: listing any
medical conditions, allergies and necessary medications.
Also if you do not own the boat, a power of attorney for the skipper is
advisable.
Also I like to have the SSB radio programed for ham frequencies or an extra
ham radio programed for SSB marine aboard (Illegal, but can be used in
emergencies--that is anyone can transmit on ham in an emergency--or a non
type certified radio can be used in an emergency)

To add to what Bob wrote, you can save major dollars by getting the
shots he has listed below from your public health agency. I won't try to
quote the actual dollars I paid for each vacination 'cuz I don't
remember. But it was on the order of one third. If my doctor wanted
$100, then I paid $30 at the public heath agency, same juice....
