In my mind for powered boats the 3 most likely things to cause a sudden end to an off shore trip, short or long, is fuel, and ummm, oh yeah, fuel, and lastly .  .  . FUEL....

Taking a marina queen offshore, say 80 miles to Bermuda, is sure to find the gunk that has been quietly lurking in the tanks for the past decade...
Getting fuel in a foreign port is another likely source (See the current Passagemaker)
Springing a fitting leak and getting bubbles is a frequent problem...
And on, and on...

If there were a way to calculate this I would guess that fuel problems would be more than half the percentage of the three top causes - whatever the other two are...
