Friends, DeSoto County, MS is undertaking a broadband expansion project using ARPA grant funds, which it will provide to internet service provides as grant subrecipients. I have seen online where numerous other states, counties and municipalities have undertaken similar projects. Many of the forms I have created so far, such as RFP and grant agreements, I have patterned from forms I have found online. Many of your jurisdictions are much further along in the process than is DeSoto. DeSoto has completed the RFP process and voted to award several grants. I'm now in the process of finalizing grant agreements. At this time, I need to prepare forms for: quarterly reporting, annual reporting and applications for reimbursements from awarded funds (invoicing). I'm trying to not recreate the wheel and any sample forms anyone is willing to provide are greatly appreciated. So far, the process has gone well, and the ISPs have been easy to work with. However, AT&T has not yet been willing to sign the grant agreement and is seeking to use a basic contract form that is not applicable.
