Can city request/require person submitting an Open Record Act request to fill out a standard "form" of request? City is receiving request via email with no address and no telephone number. While the request has a name, there is nothing to indicate whether the email is even coming from inside the U.S. Thoughts?

I have had similar problems. My (small) town and I came up with a short "request form" so that we had the necessary information to contact the individual making the request if necessary. It also helps us if someone comes back for the same information. Never had an issue, but it is a good question.

Yes, I think technically you can. See Wagner v Custer co case, issued by OK Court of Civil Appeals in Spring 2021 (not where I can access the cite). Should be helpful, involved ORA by email.
